I understand that I am establishing a virtual, limited patient-physician relationship with Paige S. Gutheil, DO (Dr. Paige) and/or a certified member of her team for the purpose of a comprehensive integrative health assessment, protocol, and consultations as described above. This will be completed over a three month period from time of enrollment. If not completed within 6 months, I may forfeit the remainder of services. I understand that this does not replace my local health care and Dr. Paige and her team are not available to evaluate acute or urgent health issues. Dr. Paige and team will offer comprehensive recommendations, communicate with my physician upon request, and prescribe supplements and/or non-controlled pharmaceuticals as needed during this period. I understand that there are ever-changing variations in state regulation regarding availability of discounted labs and prescribing regulations to local pharmacies that Dr. Paige and her team will navigate with me. These variations will not affect the value of this service package. I understand that I will have the opportunity (but am not obligated) to continue the patient-physician relationship with Dr. Paige and/or her team after the consultations are completed for continued evaluations, testing, and monitoring of supplements/medications as needed.
